Jr. High Camp is happening July 2–5 at Lakeshore Pentecostal Camp! This camp is for students going into Grades 6–8 and is an amazing opportunity to build friendships, have fun, and grow in their faith.
Students can expect a great mix of games, outdoor activities, team competitions, and meaningful services designed just for their age group.

Waiver & Medical Release Form

Each person MUST complete and sign this form before their registration will be processed. This waiver of liability is required in order to participate in Jr.High Camp Lakeshore Pentecostal Camp. This form provides a general overview of some of the risks involved in this opportunity. If for any reason you have questions or concern, please contact Pastor Alysha for more details before signing this waiver.

All trip participants are expected to conform to basic rules of conduct: No possession or use of alcohol, E-cigarettes, illegal drugs, or tobacco, unauthorized weapons, explosives; no violent behaviour.

Voluntary participation in program activities; respect for property, staff, and all program participants; voluntarily refraining from immoral behaviour as a matter of Christian faith; and voluntary compliance with event, training and payment schedules. Participants who fail to comply with these expectations may be excluded from participation at any time without recompensation of funds raised or submitted, and / or sent home at their own expense; King Street Community Church assumes no liability for any damages arising from such consequences.

Every effort will be made by our leadership team to ensure the safety and health of all participants, but it is important to note that multi-day activities involving travel to remote locations, such as this Jr.High Camp carry increased risks beyond the scope of standard youth events, such as transportation-related accidents (including, but not limited to accidents in private vehicles or municipal or private buses), outdoor-related injuries (such as falls, scrapes, lacerations, fractures, sprains, heatstroke, hypothermia, dehydration, sunburn, drowning, embedded objects, insect bites, allergic reactions or rashes), sports-related injuries (including, but not limited to, concussion, exhaustion, dehydration, impact injuries to internal organs, bruising and skin abrasions); violent crime, injury as a result of negligence (such as being hit by a car or being knocked over by impaired or unimpaired persons), food-poisoning, water-born illness, psychological trauma, death, and injuries and illnesses aggravated by delayed access to a medical facility. These risks can be further exacerbated by a participants failure to observe team safety policies and protocols. By signing this release, you acknowledge that you have (1) familiarized yourself with the details of Jr.High Camp and (2) understood the inherent risks associated with travel and ministry in a remote setting, and hereby release King Street Community Church (“the Church”), its pastors, employees, agents, volunteer workers, and the Pentecostal Assemblies of Canada (“PAOC”) along with its individual districts, employees, agents, and affiliated volunteers from any and all liability for any injury, loss, or damage to person or property that may occur during the course of your involvement, including all methods of transportation on motorized and non-motorized vehicles (for both occupational and recreational use on this trip), for all activities pertaining to ministry (including services, late nights, activities, and break-outs), and travel to and from our ministry sites, and for all activities undertaken at camp. In the event that the participant is injured or ill and requires medical attention, you agree to hold all parties free and harmless of any claims, demands, or suits for damages arising from the giving of treatment. You also acknowledge that you will be ultimately be liable for any medical care and transportation costs should the costs of those services not be reimbursed by your health insurance provider. Should the participant fail to comply with the expectations laid out in this document or if deemed necessary by the team leadership, you also acknowledge your full liability for the expenses of early transportation home, including additional airfare, chaperone / unaccompanied minor fees, and all other costs associated with carriage and transportation.

If participant is under the age of legal majority in the Province of ONTARIO (AGE 18), then as the legal guardian of the participant named on this form, I accept these terms on their behalf, and I have read and discussed the contents of this waiver with them, hereby also granting permission for said individual to participate Jr.High Calibrate Camp at Lakeshore Pentecostal Camp from July 2nd - 5th, 2026.

By checking the boxes below, you consent for the above mentioned and for youth to be transported via a chartered school bus and by existing youth leaders and their personal vehicles if needed.
